Membership Cards

Note: You may need to adjust the margins under Page Setup,
as results vary from printer to printer. You should
test your settings on photocopies to minimize waste of your membership
cards. Once your are satisfied with the results,
test your settings again by loading ONE sheet of membership cards
into your printer.
Additional
Membership Cards
In
order to receive additional membership cards, the state office must
receive dues for 75% of those already received for the current membership year. Submit
dues using the Dues
Transmittal form.
Complete the additional
membership card form and fax or mail to the state office.
